Introduction
Kenya has experienced several well-documented outbreaks of unexplained illness in boarding schools that initially prompted fears of infectious disease, poisoning or supernatural causes but were later understood primarily as episodes of mass psychogenic illness (sometimes called mass sociogenic illness). These events are important because they demonstrate that psychological distress can produce genuine physical symptoms in groups of otherwise healthy people without a shared infectious cause. The symptoms are real, even though they do not arise from a contagious pathogen.
The Kenyan cases also show why boarding schools are especially vulnerable. Students live in close quarters, share routines, face intense academic and social pressures, and closely observe one another. When an alarming symptom appears, fear, uncertainty and constant discussion can unintentionally amplify it. Rather than dismissing affected pupils as pretending or exaggerating, health specialists increasingly emphasise that these outbreaks require careful medical investigation, clear communication and psychological support.
Why boarding schools are especially vulnerable
Mass psychogenic illness is not unique to Kenya, but boarding schools provide conditions that can allow it to spread rapidly. Reviews of school outbreaks across Africa and internationally identify several recurring features:
- A genuine triggering event, such as one student becoming ill or reports of an unusual symptom.
- Close observation between students living together.
- High levels of stress, including examinations, strict discipline or personal pressures.
- Rumours that reinforce anxiety.
- Intense media attention or emergency responses that unintentionally increase concern.
- Resolution after reassurance, separation of affected groups and psychological support rather than treatment for an infectious disease.[PMC]pmc.ncbi.nlm.nih.govPMCEpisodes of mass hysteria in African schools: A study of literaturePMCEpisodes of mass hysteria in African schools: A study of literature
Researchers now prefer the term mass psychogenic illness because it better reflects that the symptoms have psychological rather than fabricated origins. People experiencing the illness are not consciously pretending. Their symptoms—including weakness, coughing, fainting, tremors, difficulty walking and breathing problems—are genuine experiences produced through complex interactions between stress, expectation and the body’s normal physiological responses.[PubMed]pubmed.ncbi.nlm.nih.govFactors related to the occurrence of mass psychogenic illness in schools: a systematic review - PubMed…
Starehe Girls and the unexplained coughing outbreak
One of Kenya’s best-known recent examples occurred at Starehe Girls Centre in Nairobi in October 2019.
The incident began when dozens of students developed a distinctive high-pitched cough accompanied by sneezing and mild fever. Concern quickly spread through the school, leading administrators to isolate affected students and temporarily close the school while investigations took place. Initial fears centred on an unidentified infectious disease.[The Standard]standardmedia.co.keThe Standard Tests find no serious disease at Starehe GirlsThe StandardTests find no serious disease at Starehe Girls - The Standard Health…
Laboratory testing by Kenyan public health authorities found only two cases of ordinary rhinovirus, the virus most commonly responsible for the common cold. The identified infections could not explain the large number of students showing similar symptoms. Mental health specialists subsequently assessed the students and concluded that the wider outbreak was consistent with mass psychogenic illness. The Ministry of Health recommended:
- individual psychological assessment;
- group therapy;
- counselling for students;
- support for teachers and caregivers; and
- greater attention to mental health and stress management within the school.[the-star.co.ke]the-star.co.keThe Star Starehe girls diagnosed with mass hysteriaThe Star Starehe girls diagnosed with mass hysteria
The Starehe case illustrates an important lesson. Medical authorities did not assume from the outset that the illness was psychological. They first investigated possible infectious causes before concluding that no disease could account for the pattern of symptoms.
Eregi Girls and the crisis of visible symptoms
A second major episode attracted widespread international attention in October 2023 at St. Theresa’s Eregi Girls High School in Kakamega County.
More than one hundred students developed striking symptoms that included difficulty walking, shaking, weakness and emotional distress. Videos circulated rapidly online showing girls apparently unable to walk normally, leading to intense speculation about poisoning, spiritual attacks, environmental toxins and other explanations.[ACI Africa]aciafrica.orgACI Africa Strange Illness at Kenyan Catholic School: What Really Happened?ACI AfricaStrange Illness at Kenyan Catholic School: What Really Happened?October 8, 2023…
Health officials admitted affected students to hospital while investigations ruled out major infectious disease outbreaks and other obvious physical causes. Although online discussion often focused on supernatural explanations, many clinicians and observers pointed instead to mass psychogenic illness, noting similarities with previous school outbreaks in Kenya and elsewhere.[ACI Africa]aciafrica.orgACI Africa Strange Illness at Kenyan Catholic School: What Really Happened?ACI AfricaStrange Illness at Kenyan Catholic School: What Really Happened?October 8, 2023…
The Eregi episode also demonstrated how modern social media can accelerate collective anxiety. Unlike earlier school incidents that spread mainly through face-to-face interaction, dramatic videos were shared nationally and internationally within hours. This broader audience amplified rumours and competing explanations long before official findings were available.
How stress, close contact and media attention amplify illness
Mass psychogenic illness follows a pattern that differs from ordinary infectious disease.
A genuine illness, frightening event or unexplained symptom often serves as the initial trigger. Once concern develops, students begin paying close attention to normal bodily sensations. Minor symptoms that might otherwise be ignored—such as throat irritation, dizziness or muscle weakness—become more noticeable because they are interpreted as signs of the same mysterious illness.
In boarding schools, several factors reinforce this process:
- students spend nearly all day together;
- friendship networks spread information rapidly;
- anxiety increases after seeing classmates become ill;
- uncertainty encourages rumours;
- emergency responses confirm that something serious appears to be happening.
Modern systematic reviews also identify heavy media coverage, misinformation and reduced trust in official explanations as important factors that can prolong outbreaks. Social media now allows rumours to spread far beyond the school itself, making reassurance more difficult.[PubMed]pubmed.ncbi.nlm.nih.govFactors related to the occurrence of mass psychogenic illness in schools: a systematic review - PubMed…
None of this means that stress simply exists “in the mind”. Psychological stress activates real biological responses involving breathing, heart rate, muscle tension and perception of physical sensations. In susceptible groups, these reactions can become contagious through observation and expectation.
Why careful investigation still matters
One danger is assuming too quickly that an unexplained outbreak must be psychogenic. Public health practice requires the opposite approach.
Authorities first investigate possible infectious diseases, environmental toxins, food contamination or other medical explanations. Only after these possibilities have been reasonably excluded can clinicians confidently diagnose mass psychogenic illness. The Starehe Girls investigation followed this sequence by conducting laboratory testing before reaching psychological conclusions.[The Star]the-star.co.keThe Star Starehe girls diagnosed with mass hysteriaThe Star Starehe girls diagnosed with mass hysteria
Equally, it is important not to dismiss affected students as dishonest or attention-seeking. Such reactions can increase distress and prolong outbreaks. Clinical guidance instead recommends calm communication, appropriate medical evaluation, limiting unnecessary alarm, providing counselling and gradually returning students to normal routines where safe.[PubMed]pubmed.ncbi.nlm.nih.govFactors related to the occurrence of mass psychogenic illness in schools: a systematic review - PubMed…
Why these outbreaks remain significant in Kenya
School illness outbreaks occupy a distinctive place in Kenya’s wider history of collective fear because they differ fundamentally from moral panics or rumours about hidden conspiracies.
Unlike the devil-worship scares of the 1990s or accusations of witchcraft, the central issue is not belief in an external enemy but the remarkable ability of stress and social influence to generate genuine physical symptoms. Nevertheless, similar social dynamics appear across these different episodes: uncertainty creates competing explanations, rumours spread rapidly, and institutions struggle to balance reassurance with serious investigation.
The Starehe and Eregi incidents have therefore become important reference points for Kenyan discussions about student wellbeing, mental health, crisis communication and the need to distinguish carefully between infectious disease, environmental hazards, supernatural claims and mass psychogenic illness. They remind us that psychological distress can become visible in profoundly physical ways, especially in tightly connected communities such as boarding schools, and that compassionate, evidence-based responses are usually more effective than sensational explanations.
