Introduction
In April 2019, a false rumour during a routine polio vaccination campaign in Peshawar triggered one of Pakistan’s most striking episodes of collective health panic. A staged video appearing to show children collapsing after receiving oral polio vaccine spread rapidly on social media. Within hours, frightened parents rushed thousands of children to hospitals, convinced they had been poisoned. Angry crowds attacked health facilities, vaccinators were threatened, and the vaccination campaign was temporarily suspended in affected areas. Subsequent medical examinations found no evidence that the vaccine had caused poisoning or serious illness. Instead, investigators concluded that the panic had been fuelled by deliberate misinformation interacting with long-standing public distrust of vaccination.[nih.gov]pmc.ncbi.nlm.nih.govMarch 11, 2021…
The episode has become a leading case study of how rumours, social media and pre-existing fears can transform ordinary bodily sensations into apparent proof of a hidden threat. It also illustrates why historians and psychologists distinguish between genuine vaccine injuries, which are closely monitored but extremely rare, and mass psychogenic illness, in which fear itself produces real symptoms and drives collective behaviour.
The staged video and the rush to hospital
The panic began on 22 April 2019 during a nationwide anti-polio campaign in Peshawar’s Mashokhel area. Soon after children received routine oral polio vaccine, a local man filmed videos claiming that pupils had become seriously ill. Other footage later showed children apparently pretending to collapse on cue while the cameraman directed them. The clips spread rapidly through Facebook, Twitter, YouTube and messaging applications before officials could respond.[geo.tv]geo.tvGeo NewsExposed: Hospitalisation of children was faked to sabotage polio vaccination driveApril 23, 2019…
As rumours accelerated, parents interpreted common complaints such as dizziness, stomach discomfort or anxiety as signs of poisoning. Hospitals in Peshawar were quickly overwhelmed as families arrived with children whom they feared had been fatally harmed. Reports described hundreds of children attending the city’s main hospitals, although many were examined, reassured and discharged within hours because doctors found no evidence of toxic exposure or vaccine-related illness.[dawn.com]dawn.comHundreds of children sick amid polio vaccine rumoursHundreds of children sick amid polio vaccine rumours - Newspaper - DAWN.COM…
The panic did not remain confined to hospitals. Crowds attacked a local health centre, set part of a clinic on fire and briefly held vaccination workers inside. The authorities suspended parts of the campaign while police investigated the origin of the videos and arrested individuals accused of deliberately spreading false claims.[dawn.com]dawn.comHundreds of children sick amid polio vaccine rumoursHundreds of children sick amid polio vaccine rumours - Newspaper - DAWN.COM…
Fear produced real symptoms without a poisoned vaccine
The children brought to hospital were not necessarily pretending to be ill. Medical researchers describe episodes like this as examples of mass psychogenic illness, sometimes called mass sociogenic illness. In such events, intense fear spreads socially, causing genuine physical symptoms despite the absence of a toxic agent.
Children who expect to become ill after hearing frightening rumours may experience nausea, dizziness, headaches, faintness, hyperventilation or abdominal pain. Once classmates display these symptoms, others may unconsciously interpret ordinary bodily sensations in the same way. The visible distress of one child reinforces the expectations of others, creating a self-amplifying cycle.
The Peshawar episode fits many recognised features of this pattern:
- symptoms appeared rapidly after alarming rumours rather than after evidence of contamination;
- medical examinations found no poisonous substance or vaccine defect;
- most children recovered quickly with reassurance and observation rather than medical treatment;
- fear spread faster than any biological illness could have done;
- social media and live television coverage reinforced the impression that a genuine poisoning disaster was unfolding.[nih.gov]pmc.ncbi.nlm.nih.govMarch 11, 2021…
The physical distress experienced by many children was therefore real, but the available evidence indicates that it resulted from fear and expectation rather than vaccine toxicity.
Why vaccine mistrust already existed
The fake video succeeded because it reinforced anxieties that had accumulated over many years rather than creating them from nothing.
For decades, conspiracy theories had circulated claiming that polio vaccines caused infertility, contained forbidden substances or formed part of a foreign plot against Muslims. In some regions, militant organisations also portrayed vaccination teams as intelligence agents rather than health workers. These rumours had already complicated eradication campaigns before 2019.[DAWN]dawn.comPakistan anti-polio drive struggles against militants, mistrustPakistan anti-polio drive struggles against militants, mistrust - Pakistan - DAWN.COMNovember 4, 2024…
Public trust suffered a major additional blow after it became public in 2011 that the CIA had organised a fake hepatitis B vaccination programme during the search for Osama bin Laden in Abbottabad. Although the operation did not involve Pakistan’s polio programme, it confirmed to many Pakistanis that vaccination campaigns could be exploited for intelligence purposes. Public health organisations, including Médecins Sans Frontières, warned at the time that such tactics endangered legitimate vaccination work by making health workers appear untrustworthy.[bmj.com]bmj.comFake vaccine campaign in Pakistan could threaten plans to eradicate polio | The BMJJuly 19, 2011…
Researchers studying Pakistan’s polio programme have argued that this combination of conspiracy theories, militant propaganda and the legacy of the CIA operation created an environment in which later rumours could spread unusually quickly.[PubMed]pubmed.ncbi.nlm.nih.govOpen source on nih.gov.
The lasting consequences
The consequences extended well beyond a single afternoon of panic.
Following the incident, many parents refused vaccination for their children, disrupting efforts to eliminate one of the world’s last reservoirs of wild poliovirus. Officials were forced to redesign communication strategies, increase community engagement and respond more aggressively to online misinformation. Investigators also concluded that the Peshawar incident involved a coordinated attempt to undermine the vaccination campaign rather than an unexpected medical emergency.[com.pk]pakistantoday.com.pkPakistan TodayFaking children’s illness in Peshawar was a conspiracy against polio drive: report - Pakistan Today…
The event also demonstrated the interaction between digital misinformation and traditional communication networks. Social media spread the original claims, while local television coverage, neighbourhood conversations and mosque announcements amplified public alarm. Once thousands of people had witnessed ambulances, crowded hospitals and distressed parents, these scenes themselves appeared to confirm the rumour, making later official reassurances far less persuasive.[firstdraftnews.org]firstdraftnews.orgFirst Draft How fake videos unravelled Pakistan’s war on polioFirst Draft How fake videos unravelled Pakistan’s war on polio
Why the Peshawar panic remains important
The 2019 Peshawar episode is remembered not because a dangerous vaccine injured thousands of children, but because a false claim persuaded thousands of people that ordinary physical sensations proved a poisoning conspiracy. It is one of Pakistan’s clearest documented examples of mass bodily alarm emerging from misinformation rather than toxic exposure.
The incident also shows that collective fear rarely develops in isolation. It drew strength from years of political conflict, damaged trust in public institutions, previous conspiracy narratives and genuine historical events that had already undermined confidence in vaccination campaigns. Understanding the panic therefore requires both immediate evidence—the staged video and medical investigations—and the wider social history that made so many parents ready to believe it.
